§ 01 Client Document Intake

The business intake dossier.

Fill in what you can. Upload what you have. The office will review the file and reach out — we've kept the form long on purpose, because the more we know up front, the less we have to ask later.

Step 1: Identify the business

Tell us about the entity that's filing — and who's authorized to speak for it.

Business entity

Authorized signer / primary contact

The person who can sign on behalf of the entity — owner, officer, member, or other authorized representative.

Contact preferences

How should the office reach the business — and which contacts get routine updates?

If the office already started this file, or if you uploaded before, enter that Submission ID and we'll keep everything under one record.

Case profile (optional)

These details help the office tailor the document checklist and the initial review.

Only check what clearly applies. Skip if not sure.

Just upload everything

Tap below and pick all your documents at once — pay stubs, ID, taxes, anything you have. Our team will sort them after upload.

Step 2: Upload by category

Fifteen categories. Each has its own drop area. Drag files from your computer, paste screenshots, or tap “Take photo” from your phone. Maximum per submission: 150 files and 120 MB total (30 MB per file). Skip any category that doesn't apply.

You do not need every document before you begin. Upload what you have now and use the same Submission ID later for the rest.

Step 3: Mark missing items

Use the dropdown even if you have not uploaded anything in that category yet. That helps us tell the difference between “not available yet” and “still being gathered.”

Entity formation documents

Articles of incorporation, certificate of formation, operating agreement, bylaws, partnership agreement, EIN/SS-4 letter, and any state filings (annual reports, certificates of good standing).
